Hopewell Health Centers, Inc. is seeking an Adult Community Psychiatric Supportive Treatment Team Leader.

CPST service provider delivers quality, person and family-centered, medically necessary, rehabilitative, culturally-competent Community Psychiatric Supportive Treatment interventions including but not limited to: assessment of needs, skills building/empowerment, crisis management and intervention, advocacy, education/training, eliminating barriers, coordinating/linkages, and monitoring.

Adult CPST Team Leader Duties:
- Ensure that all cases are seen and followed in a timely manner in accordance with clinic access guidelines/clinical need.
- Assign cases/reassigns cases.
- Facilitates team meetings on a regular basis.
- Reviews cases on weekly basis with staff and as needed with Administrative Supervisor.
- Provides clinical support as needed.
- Interfaces with Adult/Child teams/Respite/Hocking St/430 W. Union St/Blue Line Dr./Sami Housing and any contract partners to insure that client needs are being met.
Liaisons with community partners: GP, AMHA, 317 Bd, Athens Homeless Coalition, ISS, HRS, Medical community, SSA, JFS, food banks and etc.
- Clinical oversight of ACPST Team, including but not limited to:
- Complete yearly clinical evaluations.
- Directs/provides emergency backup to ACPST providers.
-Reviews applications for housing assistance in the form of vouchers, special programs, start-up funding, loans and etc.
- In collaboration with person served and/or family/legal guardian, develops an individualized service plan (ISP) for each person in the program to achieve behavior change, increased quality of life, and the best outcomes possible for the people we serve. Generates a current, legible integrated clinical record (ICR), in which documentation reflects a clear clinical picture for assigned person served.

Background Profile:
- LSW, RN, or LPC
- Bachelor Degree in social work or closely related field (mental health)
- Valid driver’s license, automobile insurance, and good driving record
