Job Description:  Clinical Supervisor of Seeds of Hope Medicaid Subcontract

Overview:  Will serve as the part-time staff member responsible for overseeing Hopewell Health Center’s Medicaid sub-contract with Seeds of Hope.

Licensure Needed: LISW, LPCC or psychologist

JOB DUTTIES: 

I. This staff person will be responsible for:

  1. Training and supervising Seeds of Hope clinical support staff providing services at Seeds of Hope.
  2. Serving as the primary liaison to Hopewell Health Centers (HHC) and Seeds of Hope.
  3. Educating Seeds of Hope staff, clients, and administrators about the role and limits of on-site services.
  4. Providing training programs, as required for staff to bill for mental health services or as requested, on topics of interest to the student body of Hocking College, such as: Depression awareness, sexual assault prevention, non-violent relationships, assertive communication, etc.
  5. Ensure that Seeds of Hope staff complete documentation in a timely, accurate and medically necessary way.
  6. Monitoring and tracking the progress of clients in the program, and referring to psychiatry or to crisis intervention or other services as appropriate.
  7. Provides quality, person and family-centered, medically necessary, culturally-competent assessment and treatment planning face-to-face with individuals, couples, and groups, to include, but not limited to: diagnostic assessment and treatment of mental, emotional and substance abuse disorders.
  8. In collaboration with the client and/or family/legal guardian, develop an individualized service plan (ISP) for each person in the program to achieve behavior change, increased quality of life, and the best outcomes possible.  Generate current documentation which reflects a clear clinical picture for the client served
  9. Complete correctly and submit in a timely manner, per agency policy, documentation, billing, time sheets, mileage, and, as required by supervisors, work and appointment schedules.
  10. Attend as scheduled, and participate in supervision, team meetings, professional development, training, and serve on agency committees, as appropriate.
  11. Abide by agency code of conduct, policy and procedures, and mission statement (e.g. business, clinical, personnel, and plan manuals) at all times on agency property and/or while conducting agency business.
